No, you don't have to create an account in this website to sign in. You have to use your Tesla credentials
and if you want to obtain a subscription then you need an account here.
No, you don't have to pay twice for the app. This is a sync delay from Google after purchasing
the app. Go to the
Google Play Store and try to install the app on your watch or phone from
there.
If you already paid, Google is going to process only one payment and cancel the rest, you don't
have to worry.
No, this app only works with smartwatches that runs on WearOS and have the Google Play Store.
After you subscribed to the prime membership open the app on your watch and click
go to Settings > Premium > Verify Premium enter the same email address that
you used to subscribe and press verify. You should be able to use the premium features.

Try forcing a token refresh from Settings > Account > Refresh Token.
If that doesn't work try logging out and login back in.
This is the intended behavior. The app is stateless. It is not aware of the current state of the vehicle.
The reasoning behind it is that it helps to send commands faster than waiting for the car to wake up and then send a command.
The app will wake up the car and execute the command right after. You have all other commands inside the More Options menu item below the main screen.
Each command with a unlock/lock state and on/off state has it own command.
